The Northern Health Trust is rolling out a new service model, that they believe will be an improved, more efficient service for their patients.
People in the community will have access to a dedicated, local team.
Roy Hamill, the Assistant Director of Community Care says this new service will give better outcomes for service users:
Areas in the Trust such as; Antrim/Ballymena, Causeway, East Antrim and Mid Ulster will have access to this.
The service will support people in their own homes by identifying those at greatest risk of hospitalisation.
An intervention and support care plan will then be put in place for those at the greatest risk.
It's part of work underway within the Trust, to ensure patients can access services in their local community and acute services are retained for those in need of urgent care.
